Appointment request form for WordPress

Not every business needs a booking calendar with open slots. Often it's enough for customers to name a preferred date and for you to confirm it.

Last updated:

The fields

  • Name (required)
  • Email (required)
  • Phone
  • Preferred date (required, date field)
  • Time of day (morning, afternoon)
  • Anything we should know?
  • Consent to data processing

Built in 5 minutes

  1. Add the “Flinkform Form” block.

  2. Pick the “Appointment request” template.

  3. Set an earliest and latest date on the date field if you need to.

  4. Publish the page and turn on the confirmation email to the customer.

Tip

To be honest: Flinkform doesn't do open time slots or calendar sync. That's what booking plugins are for. For a request you confirm by email, this template is all you need.

Block markup to copy

In the block editor, open the three-dot menu at the top right, switch to the code editor, paste the markup, switch back to the visual editor and save. That's your form, with Flinkform 1.15.0 or later.
